The Population Health Coordinator is a member of a multidisciplinary team. The Population Health Coordinator position serves as an advocate and educator. The Health Coordinator’s primary function is to ensure the patient remains compliant to the treatment plan. Navigators achieve this goal by building relationships with patients and physicians, coordinating the plan of care, assisting with appointments, transportation needs, education, resource provision and/or representation within the multidisciplinary care environment. The Population Health Coordinator reports directly to the RN Director of Quality Management.

The Medical Assistant Care Coordinator is responsible for patient care within the setting of the clinic environment.  The Care Coordinator position also assist patient with managing their health care needs remotely (via telephone, or video calls).  They are trained and capable of performing patient care for the age groups newborn to 105 years old.   The Medical Assistant Care Coordinator reports directly to the RN Director of Clinic Nursing.

The Clinical Education Specialist serves as the receptionist for Clinical Education. Stationed at the entrance of the building, this person directs patients and staff to scheduled locations. This position also works closely with Diabetes Education, maintaining records and scheduling patients, is a chairman of the DAISY award, coordinates and submits courses to HealthStream and assists with trouble shooting. This position also maintains all AHA (American Heart Association) courses and submits to the AHA.   The Clinical Education Specialist reports to the Clinical Education Coordinator and to the CNO.

The Clinic LPN/MA is responsible for patient care within the Clinic, trained and capable of performing patient care for the age groups newborn to 105 years old. The Clinic LPN reports directly to the Clinics Operations Director.

The DME Intake Specialist greets customers, provides retail sales assistance and general office support for Bothwell Medical Equipment. In addition, this individual ensures that the intake process is accurate, complete, and timely.

Responsible for direct patient care and the assisting of the physician or mid-level practitioner.      This position will report directly to the Clinic Site Manager.

The Patient Registration Supervisor manages the daily operation of the department, is responsible for maintaining staff schedules, assists the Director in department management processes, maintains interpersonal relations and works to meet the job-related needs of the employees, facilitates problem solving and resolution, and provides education and guidance for staff.   The Patient Registration Supervisor reports directly to the Director of Financial Services.

The Clinic Coder 3 is responsible for assigning appropriate Evaluation & Management CPT coding, ICD-10-CM diagnosis and CPT surgical procedure codes for services rendered in the clinic setting, inpatient and outpatient hospital surgical setting. Also abstracts clinical and demographic information into clinic and financial databases.   The Clinic Coder 3 reports directly to the Director of Clinic Financials.

The Centralized Scheduling/Auth Referral Specialist – Medical Assistant is responsible for the following: answering calls coming into the call center, scheduling all Clinic offices for daily appointments, scheduling hospital tests excluding surgeries, & ensuring authorizations are obtained and documented.   The Centralized Scheduling/Auth Referral Specialist – Medical Assistant reports directly to the Patient Financial Services Director.

The Emergency Care Technician must have the skill and knowledge to provide population specific care.  He/She must have the ability to identify the needs and requirements of the patient and report findings appropriately to the nurse.   The Emergency Care Technician role is guided by the tasks described on the Competency Checklist and does not include the primary nursing functions of assessment, evaluation, judgment, and delegation.   The Emergency Care Technician reports directly to the Charge Nurse.

The RN / GN must have the skill and knowledge to provide population specific care.  He/She must have the ability to assess, evaluate, identify, and interpret the requirement and needs of the patients and perform based on those findings.    The RN / GN reports directly to Charge Nurse/Manager/Director.

The Charge Nurse is accountable for excellence in the clinical practice of nursing and the delivery of patient care.  He/She directs and oversees function and activities of the assigned unit.  He/She recommends, develops and implements policies, procedures, standards and regulations for personnel, patients, medical staff and the public.  He/She assumes the duties of the Nurse Director in his/her absence.   The Charge Nurse reports directly to Nurse Director.  

The Clinic LPN is responsible for providing population specific care within the outpatient setting.  Performs a broad range of nursing, office, and laboratory duties to provide exceptional health and wellness services to those from birth to 105 years of age.   The Clinic LPN reports directly to the Director of Clinic Operations   MA Responsible for direct patient care and the assisting of the physician or mid-level practitioner.       This position will report directly to the Clinic Site Manager.

The Clinic Receptionist performs duties directly involved with all aspects of the secretarial duties needed to maintain medical records. The Clinic Receptionist handles all incoming telephone calls, directing calls to the appropriate department or person and takes messages when needed. The Clinic Receptionist assists the Patient in checking in for their appointment and for checking the Patient out after the appointment, follow-up appointment scheduling.   The Clinic Receptionist reports directly to the Clinics Operations Director

The staff occupational therapist is responsible for the evaluation of patients; development, coordination and implementation of the treatment plan; discharge planning, maintenance of informative, accurate and timely documentation. The Occupational Therapist is responsible for supervision of the Certified Occupational Therapy Assistant (COTA).   Based on assigned unit/area, the occupational therapist could be involved in team meetings, family meetings, home evaluations, staff training and basic office duties/reports.  Treatment outside the hospital setting could include home.   All staff occupational therapists are required to keep current national registry and Missouri licensure, in addition to participating in continuing education and training of others.   The staff occupational therapist reports directly to the Director of Rehab.

The Radiologic Technologist-Not Registered performs radiographic procedures at a technical level not requiring constant supervision. The technologist performs a variety of technical procedures that will require independent judgment, with ingenuity and initiative to apply ionizing radiation to produce diagnostic radiographs. Assumes responsibility for designated areas of procedures as required. The individual is trained in a certified Radiology Technology program and capable of performing patient care for the age groups: 0-105 years old. The Radiologic Technologist Registered reports directly to Radiology Chief Technologist
