The Population Health Coordinator is a member of a multidisciplinary team. The Population Health Coordinator position serves as an advocate and educator. The Health Coordinator’s primary function is to ensure the patient remains compliant to the treatment plan. Navigators achieve this goal by building relationships with patients and physicians, coordinating the plan of care, assisting with appointments, transportation needs, education, resource provision and/or representation within the multidisciplinary care environment. The staff occupational therapist is responsible for the evaluation of patients; development, coordination and implementation of the treatment plan; discharge planning, maintenance of informative, accurate and timely documentation. The Occupational Therapist is responsible for supervision of the Certified Occupational Therapy Assistant (COTA).   Based on assigned unit/area, the occupational therapist could be involved in team meetings, family meetings, home evaluations, staff training and basic office duties/reports.  Treatment outside the hospital setting could include home.   All staff occupational therapists are required to keep current national registry and Missouri licensure, in addition to participating in continuing education and training of others.   The staff occupational therapist reports directly to the Director of Rehab.
