The Team Lead Case Management is an allied health care practitioner who functions as supervisor to the staff. This individual is responsible for the performance of all procedures and tasks assigned or ordered for completion. He/She is to assist the director in supervising human, fiscal, and other resources as needed to manage case management and social services practice and patient care. Assists with and/or accepts collaborative responsibility for developing, implementing and interpreting policies, procedures, standards and regulations for personnel, patients, medical staff, and the public.
The Team Lead Case Management reports directly to the Director Case Management.
1. Ensures hospital compliance with professional, regulatory, and government standards of care.
2. Ensure the satisfactory completion of all department clinical procedures, by developing and implementing strategies that maximize the effectiveness of the medical department. Including coordination, planning, and conducting of patient care and by being a resource to staff.
3. Organize, supervises, evaluate case management and social service activities by anticipating and maintaining staffing pattern for the departments, meeting the patient need for continuity of care and level of care by facilitating and implementing the use of multidisciplinary team approach. Also demonstrates knowledge of needs for specific units and patient level of care and status.
4. Evokes in staff sense of responsibility and accountability for their work and continuous improvement of their performance. Also hold self-accountable to same standards.
5. Participates in committee and task force functions as assigned.
6. Demonstrates knowledge of budgetary guidelines for the departments and assists the director in maintenance of adequate supplies and resources within those guidelines.
7. Performance evaluations, work schedule assignments, salary increase recommendations, clinical competency assurance, time card maintenance, and mandatory in-service attendance of the staff.
8. Performance of Team Lead Case Management duties as needed.
9. Perform other duties as assigned.
1. Education:
a.) Bachelor’s degree in Nursing from an accredited School of Nursing or related field. Master’s degree preferred.
2. Licensure/Certification:
a.) Must have a valid, current Missouri RN license.
3. Work Experience:
a.) Minimum of two years’ experience as a staff nurse.
